Purpose. Completed by the work crew before a non-routine or high risk task to break the task into steps, identify the hazards at each step and agree the controls.

1.When a job safety analysis is required

A job safety analysis must be completed before hot work outside a welding bay, lifting with the crane where the load is unusual or exceeds half the rated capacity, working at height on a trailer or bus roof without a platform, entering a confined space, and any task that has no work instruction. The supervisor decides whether other tasks require one.

2.Completing the analysis

The crew completes the form together at the work location. List the steps in order, identify what can go wrong at each step and record the control that will be used. Controls must be chosen from the hierarchy of control, preferring elimination, substitution, engineering and isolation before administration and personal protective equipment.

3.Permits and isolation

Where the task requires a hot work permit, a confined space entry permit or an energy isolation, record the permit number on the form. The task must not start until the permit is issued and the isolation is verified.

4.Sign on

Every person taking part in the task must read the analysis and sign on. A person who joins the task later must be briefed and must sign on before starting. The supervisor must review and approve the analysis before work begins.

5.Review

Stop the task and review the analysis if conditions change, a new hazard appears or the method changes. File the completed form with the work order.

6.Complete the form
